Spiritual Care Coordinator- Minister/Chaplain

Friendship Haven is searching for a key member for our campus Wellness Team with the primary responsibility for the Spiritual Wellness of our campus residents. The Coordinator oversees all aspects of worship services, including facility accommodations, worship supplies and coordinating volunteers for various ministries. Key traits include ability to deliver excellent customer service and public relations, ability to empathize, show compassion and have strong organizational skills. Proficiency in writing and public speaking and computer usage.                                                                                                                                                                                                                        Preferred Requirements:

Education: Seminary graduate degree

Experience: Three years experience as a minister/chaplain

River Ridge Assisted Living- Part-Time

River Ridge/Second Family Caregiver: Part-Time- 10:30PM - 6:30AM. Every other weekend/every other holiday.  This could be a weekend only position as Fr/Sat/Sun nights. This position covers resident cares and basic first aid for River Ridge along with Kenyon Place and the Town Homes. There are some laundry and housekeeping responsibilities also. This position does have scheduled visits for medication pass, toileting reminders/support and AM/PM dressing. The individual chosen will need to demonstrate a skill level for nurse delegation for medications and personal cares along with critical thinking skills. Certified Nursing Assistant Preferred but not required if willing to test out for certification. River Ridge positions include cross training to house keeping, kitchen service and hostess, also.

Full-Time Care Transition Coordinator- Skilled Neighborhood

Purpose of the Position:

A unique and versatile position in the Skilled Nursing/Catalyst Neighborhood of the Friendship Haven care facility.

Essential Duties:

  • Responsible for coordinating the care needs for the Skilled Nursing neighbors from admission to discharge.
  • Complete admission/discharge paperwork for each resident, making necessary discharge referrals, identifying and implementing education/teaching in preparation for discharge and other miscellaneous duties relating to discharge needs.
  • Accountable for completion of identified MDS interviews/sections, CAAs and development of associated care plans.
  • Set up and attend care plan conferences for the Skilled Nursing neighbors.
  • Supervises homemakers/activities personnel in the Skilled Neighborhood, including scheduling and timecard management. Supervises and mentors skilled Nursing staff.
  • Provides education and performs care reviews to neighborhood team members.
  • Assists Clinical Coordinator with neighbors/family/staff concerns as needed.
  • Assists neighborhood in day-to-day nursing staff vacancies as needed and able.
  • Assists in other various opportunities as able and necessary to meet the care needs of the Skilled Care neighborhood.
  • Is required to be on call in the Nursing Administration rotation schedule.
  • Accountable for planning and implementing activities as needed in the Skilled Neighborhood.
  • Provide Social Services such as one-on-one counseling, depression screening, reassurance and mental health referrals as needed.
  • Explain and provide required Medicare notices per federal and state regulations to Skilled Neighbors.
  • Work with various outside Agencies as needed.
  • Provide financial counseling as needed by residents or family.
  • Develop and update educational materials that can be utilized for teaching and provided to the Skilled Neighbor upon discharge.
  • Must have knowledge of CPR and IPOST.
  • Serves on Friendship Haven Committees as needed.

Gardens Assisted Living- Full-Time RN

Hours: Monday- Friday- Day Hours

Position: Full-Time Salaried position

Purpose of the Position:

 

Participates in assessing, planning, organizing, directing, delegating and facilitating the clinical care and services provided in the Gardens Assisted Living.  This position assesses and evaluates functional abilities, health history and cognitive status of residents and participants.  For residents who require personal and health related care and services this position is responsible for the evaluation, coordination, supervision and delegation of personal and health related services and care provided by staff.  Partners with the Director of Assisted Living for responsibilities to the selection and supervision of employees, staff training and education, delegation of services and ensuring the competency of staff providing personal and health related services and care.

Essential Functions:

 

  • Performs nurse delegation and assures health and personal care provided in the Gardens is consistent with Iowa Code Chapter 231C and Administrative Rules Chapters 6, 67, 69 and 70, and other regulations therein referenced in order to maintain state certification.
  • Provides case management to assure the Gardens residents are receiving the services they need and are appropriate for continued Assisted Living Occupancy.
  • Follows Friendship Haven policies and procedures.
  • Participates in the development, implementation and review of policy and procedures specific to Assisted Living.
  • Coordinates the Gardens interviews and makes the selection of new staff to ensure that quality, and services are provided to residents.
  • Serves on committees as requested.
  • Supervision of services provided by direct care givers in cooperation with the River Ridge Residential Nurse and Director of Assisted Living.
  • Conducts resident evaluations, service plans and nurse reviews within appropriate time frames as needed.
  • Participates in resident and family orientation upon residents assuming occupancy in the Gardens.
  • Addresses the health and personal care needs of residents and keeps family members informed.
  • Provides education to residents and families related to health care and aging issues.
  • Provides health and personal care and age-related education and training to staff, and the general public as requested.
  • Participates in the planning and development of the operating budget and maintains fiscal accountability in areas related to nursing expenditures and supplies. Participates in the development, implementation and evaluation of strategic plans, goals and objectives for the Gardens.
  • Provides oversight and makes necessary requests in order to preserve the physical appearance of the Gardens neighborhood.
  • Responsible to communicate with Medicare providers on the resident’s behalf.
  • Provides fall/incident reviews and follow up as necessary.
  • Provides off hour support to delegated staff for resident care. Commonly by phone.
  • Provides back-up nursing and assistance in River Ridge/Second Family in the absence of the River Ridge Residential Nurse.
  • Provides assistance to administer TB and flu shots in Assisted Living and with employees.

 

Knowledge, Skills and Abilities:

 

  • Knowledge of a social model of service delivery resident centered care.
  • Knowledge and understanding of Iowa Code Chapter 231C and Administrative Rules Chapters 6, 67, 69 and 70.

  • Sits, stands, bends, lifts and moves intermittently during work hours.
  • Willing to work beyond normal working hours, and on weekends and holidays when necessary.
