Purpose of the Position:

The Lead will be responsible for the neighborhood operating smoothly, promoting rich programming to enhance the residents’ wellbeing, and ensuring a high level of customer service and satisfaction.  The Lead will collaborate with the Coordinator, residents and employees to determine what support services the neighborhood needs to live with faith, passion and purpose.

Neighborhood Leadership Functions:

  • Provide leadership and vision to the household by modeling professionalism, customer service, collaboration, flexibility and acts as a coach to keep processes resident centered.

  • Responsible for assisting with schedules for all assigned staff along with bi-weekly timecard processing, as delegated.

  • Works cooperatively and respectfully with residents, co-workers, families, visitors and volunteers to create a warm and positive environment in the home.

  • Build relationships with residents, families and guests and plays a role in managing resident and family complaints and communicating with the Coordinator, as needed.

  • Maintain a safe, comfortable, functional environment by assessing the household for sanitation and potential hazards, implementing practices utilizing disaster preparedness and emergent care, investigating and reporting all incidents and accidents, observing the team for safe work habits and addressing observations.

  • Facilitates household’s ability to plan and carry out personal interests and social activities for residents and the neighborhood.

  • Encourages a climate of continued learning and personal growth for all.

  • Facilitates each household’s ability to plan and meet the psychosocial needs of residents and the household.

  • Ensure household meetings (resident, employee and neighborhood) are taking place appropriately, engaging all residents and employees.

  • Attends or assures the household is represented, as delegated, on all appropriate councils and teams of the community.

  • Encourages extended family participation.

  • Notifies and coordinates with appropriate parties when transition or deep cleaning is needed.

  • Assist Coordinator with tracking, trending and auditing, as delegated.

  • Ensure knowledge of all computer systems such as EHR and payroll software.

  • Responsible for scheduling appointments with all external/internal agencies and organizing transportation to and from, if needed.

  • May be required to fulfill on-call responsibilities.

Health Information Functions:

  • Scanning, photocopying and faxing, as needed, to ensure a medical record is complete.

  • Closed medical records completion.

  • Appointment scheduling and transportation scheduling.

  • Other tracking, reports and duties, as necessary.

  • Complete pharmacy charges paperwork.

  • Complete chart close out.

Care Plan and Documentation Functions:

  • Responsible for admission, transfer and discharge assessments, documentation and reporting.

  • Incorporates the collected assessment date into the care plan.  Participate in care plan conferences for all household residents, as delegated.

  • Along with the Coordinator, investigate and report all incidents and accident, as needed.

  • All other duties as assigned.

Knowledge, Skills and Abilities:

  • Knowledge of a resident centered model of service delivery within neighborhood.

  • Ability to work effectively with co-workers in a team environment.

  • Proficiency in writing and communicating with other team members.

  • Accountability to complete tasks with minimal supervision.

  • Ability to deliver excellent customer service and public relations. Ability to be tactful in resolving customer service concerns and situations.

  • Understanding of social services, housekeeping, food service/sanitation and activities programming

Minimum Requirements:

  • Education:  Level of education to communicate and lead effectively.

  • Licensure: Must be willing to obtain CNA certification, and willingness to learn social services, MDS, Activities, environmental services, food service and all other support services functions.

  • Experience: Must demonstrate leadership qualities and initiative in primary job function.

Physical Demands:

  • Sits, stands, bends, lifts and moves during work hours.

  • Willing to work beyond normal working hours, and on weekends and holidays when necessary.

Mental Demands

  • Subject to frequent interruptions.

  • May be subject to hostile and emotionally upset residents, families and visitors.
