Purpose of the Position

The Social Services Coordinator will assist the Director of Social Services in the day-to-day operations of the social services needs in the Simpson Health Center. The program is intended to meet the psychosocial wellbeing and needs of our residents. The Coordinator will help the Director lead all efforts to ensure successful transitions throughout our continuum.

Social Services Leadership Functions

  • Assist the Director of Social Service in the planning, developing, organizing, implementing, evaluating, and directing of social services programs of this facility.

  • Responsible for scheduling, communicating and attending care plan conferences for ICF residents, along with serving as point of contact for resident families.

  • Responsible for discharge follow-up for skilled residents.

  • Audit quarterly charts and care plans per the MDS schedule.

  • Ensure family requests from care plan conferences are completed as requested on a timely basis.

  • Update the social services assessment and care plan when events, or changes, occur for ICF residents.

  • Provide campus tours for prospective residents and families.

  • Communicate and manage application process and waitlist

  • Responsible for referrals to SNF level of care in the absence of the Director of Social Services.

  • Administrative duties, as assigned by the Director of Social Services.

  • In the absence of the Social Services Director, provide social services assistance to campus, as needed.

  • All other duties as assigned.

  • Accurately and completely document social service actions and interactions in each resident’s medical record.

  • Assist with seamless transfer of cross-campus referrals.

  • Knowledge of a resident centered model of service delivery within neighborhood.

  • Proficiency in writing and communicating with other team members.

  • Maintains confidentiality of protected health information, including verbal, written and electronic communications.

  • Ability to deliver excellent customer service and public relations. Ability to be tactful in resolving customer service concerns and situations.

  • Understanding of social services, housekeeping, food service/sanitation and activities programming.

Minimum Requirements

  • Education: Degree in Human Services or related area.

  • Experience: Must demonstrate leadership qualities and initiative in primary job function.
